The coordinator services are delivered by a Transition Coordination Agency (TCA). The purpose of this service is to help an individual relocate to a community-based setting upon discharge from a long-term care facility. Transition Coordination Services may also include the provision of items necessary for members to effectively establish a community-based residence. This includes moving expenses, security deposits, utility deposits, one-time pest eradication service, and items purchased. If you or a loved one wants to live independently outside of a care facility, please set an appointment so we can discuss your specific needs and begin your care plan.
