What is Targeted Case-Management –Transition Coordination?

Transition Services assist Health First Colorado members who qualify to live effectively in their own communities. Transition Coordination is a transition service for people who want to move out of a long-term care facility. Transition Coordination helps members to move successfully and safely into their own home. It can also help you find housing if you might need it. TCM services help members to become more independent and, as a result, incur a far lower cost to the state for members to live in the community compared to LTC facilities. Residents of LTC facilities can access community living options through the Options Counseling process. They may be referred to Options Counseling by family/friends, community members, providers, LTC social services, and/or through the Minimum Data Set (MDS) Assessment administered quarterly within LTC facilities. Services are provided during the transition process and the first year of community living, allowing clients to better adjust to their new living situation.
